Die Diabetesfachberaterin erhebt die persönliche Anamnese der KlientInnen, die individuelle Beratungsplanung und führt Beratungen und Coachings durch:
Gemeinsames Festlegen der therapeutischen Ziele, Überprüfen des Selbstmanagements und der Zielerreichung
Vermittlung theoretischer Grundlagen über:
- Stoffwechsel
- Stoffwechselselbstkontrolle (Blutzuckerkontrolle, Urinaceton-Kontrolle Typ 1, Blutdruckkontrolle)
- Orale Antidiabetika / Insulintherapie
- Rauchstopp bei Diabetes
- spezielle Diabetesformen
